What is Adult Foster Care?​

The Adult Foster Care Program is a MassHealth program that provides people aged 16 and older with nursing oversight, care management services, and a caregiver who lives with you and provides supervision and support with bathing, dressing, toileting, transferring, ambulating, eating, laundry, shopping, housekeeping, meal preparation, medications management, and transportation.

 

Who can be a caregiver?

Your caregiver could be your son, daughter, grandchild, niece, nephew, friend, etc. However, your caregiver cannot be a spouse, parent of a minor member (including adoptive parent), or any legally responsible relative. Your caregiver must have the ability to make mature and accurate judgments and with no mental, physical, or other impairments.

 

You may be eligible for the Adult Foster Care program if you:

  • Have MassHealth Standard
  • Have CommonHealth
  • Enroll in a Senior Care Options program

AND

  • Because of a medical or mental condition, you require daily physical assistance or cueing and supervision with at least one of the following: bathing, dressing, toileting, transferring, ambulating, or eating
  • Your doctor prescribes services for you
  • You and your caregiver live together in a qualified setting
  • You are 16 years of age or older
  • Your caregiver is 18 years of age or older

 

Who pays for Adult Foster Care services?

GLSS will pay your caregiver for providing you with daily assistance.

An Older Woman with a Walker and a Younger Woman

How we help

Susan is a 66 year old elder who has multiple medical conditions, including dementia, fibromyalgia and depression. She also had a kidney transplant 6 years ago. The elder lives with her daughter Michelle and son-in-law Mike in a private home. The elder needs daily assistance and supervision with bathing, dressing, ambulating among other activities of daily living. The elder follows a special diet and needs encouragement to eat throughout the day.

The elder’s daughter and son-in-law have been assisting the elder for many years and wanted to see if they can receive financial assistance, especially since the daughter is providing full-time care to the elder at home. They called GLSS and a nurse and a care manager came out to assess the elder for Adult Foster Care (AFC) services. In addition to being approved for AFC services, the elder also receives companion services to provide the caregiver with respite.

Today, the elder continues to live safely at home with her daughter and son-in-law. The daughter is paid a stipend for providing care to the elder. The elder also receives nursing oversight and care management services from GLSS on a monthly basis.
