Starting in fall 2013, MassHealth and Medicare joined together with health plans in Massachusetts to offer One Care to consumers who are dually eligible for MassHealth and Medicare. Since that time, GLSS has been offering Long-Term Services and Supports (LTSS) Coordination to people age 21-64 in 19 cities and towns in Essex County, from Lynn to Rockport. The goal of One Care is to offer a better, simpler way for people with disabilities to get all the care they need in one plan. The program is offered by Commonwealth Care Alliance (CCA) in Essex County.
LTSS includes things that help people do activities, like a wheelchair or a device that help with communication; provide assistance with activities of daily living, like bathing and getting dressed; and programs that provide daily living skills, such as day programs or adult foster care.
GLSS' team of specially trained LTSS Coordinators are currently serving over 500 consumers.
