Community Transition Liaison Program

Support for nursing home residents who want to return to the community.

The Community Transition Liaison Program (CTLP) is for people who are currently residing in a skilled nursing facility and have expressed interest in returning to the community.  Residents of nursing facilities often have complex, but sometimes not insurmountable barriers to discharge. These barriers can include availability of appropriate/affordable housing, access to required level of care for a new environment, required documentation, and other factors. 


Community Transition Liaisons work with individuals (or their family members) to create a personalized plan for discharge from a skilled nursing facility. We work with people to find the appropriate living situation, whether that is returning to a previous residence or finding a new affordable option. We also connect people with services and supports to help them adjust and thrive upon returning to the community. Our Liaisons meet with residents to discuss their needs and provide options for a safe plan to return to community living, assist with applications for housing and public benefits including collecting all necessary documentation, and coordinate with state and community agencies to identify resources and make referrals.

Who is this program for?

The Community Transition Liaison Program (CTLP) is a free service available to all short- and long-term skilled nursing facility residents who are 22 years old and older, regardless of insurance type, who are in interested in transitioning back into the community. Nursing facilities served include those in Lynn, Nahant, and Saugus.
