If you or someone in your family is 60 or older, we make it easy for you to get exactly the services you’re looking for.  With just one phone call, our Information & Referral Specialists will help you tap into a vast array of information about programs, services, and community resources that can help you or your family members.

The first time you call us, you’ll speak with someone in our Information & Referral department. Our friendly, well-trained staff will answer all your questions about services and benefits. We’ll help you find home care, meals, transportation, an advocate…whatever you need.

Who is this program for?

Information & Referral is a free, state-funded program that is available to all residents, whether you live in Lynn, Lynnfield, Nahant, Saugus, or Swampscott, or have a family member who lives in one of our five communities. If you are seeking information outside of our area, visit 800ageinfo.com to find the Area Agency on Aging offering this service in your community.

How does this work?

Depending on your needs, one of our Information and Referral Specialists will answer your questions and provide you with information about a broad range of community supports and resources.  If it is determined that services through GLSS are needed, an appointment will be scheduled with one of our Intake Specialists, who will visit you in your home to gather more information.  If approved for services, you and your GLSS Case Manager will develop a specialized plan of care unique to you and your unique needs.

Referral Form for Professionals

If you are a professional and would like to make a referral to GLSS, we have a fillable form available. There are three ways to fill out the form.

1. Download this referrer fillable form and open with Adobe Reader. You can type in the boxes and fill it out with your keyboard and then print it and fax it or save it with a new name and mail it through a secure email service. 

2. Open the link using the Chrome browser (Edge, Explorer, and Firefox will not work). You can type in the boxes and fill it out with your keyboard and then print it and fax it or save it with a new name and mail it through a secure email service. 

3. Open the referrer fillable form link, print it, and fax it to the number on the top of the form.
